Client competent with ADLs & IADLs Client managing chronic diseases, cancer recovery/remission &/or debilitation post acute injury/illness desiring to actively practice arresting/improving/reversing symptomatology &/or conditions Client desiring to experience a directed coaching program customized to effect change from the inside out, transform and improve quality of life Progress Tracking: Initial plan of care & progress notes with optional medical coordination, monthly goal sheets, monthly “Glow” sheets, Point of Care assessments for “VitaCheks” pertinent to symptoms and conditions and improvements in recovery, strength, range of motion, balance Medical Coordination: Optional liason with health care provider to identify points of weakness, contraindications, trajectory starting point/escalation and integration of suggested dietary and fitness best practices |
